1. Seaman Apprentice RDX did so negligently discharge her automatic grenade launcher directly at her Commanding Officer LT Havercruft in the evening of 5 May 1966. Her wanton disregard for

the saftey of her team, willful negligence in weapons handling, and dastardly act to injure her leadership goes against the good order and discipline of a military unit.

2. Seaman Apprentice RDX will receive the following administrative actions:

a. Seaman Apprentice RDX will surrender herself to the Master-at-Arms at 0600 6 May 1966 to be confined to the brig for no shorter than 5 days.

b. SA RDX is restricted to bread and water for 5 days.

c. SA RDX will forfeit all pay and allowances for 1 month.

d. SA RDX will retain her rank, but be placed on a control roster and receive a referral performance report.

3. SA RDX is entitled to submit a rebuttal in written form within 5 days from this date to Battalion JAG.
